I understand that all the activities and events including, but not limited to, sporting competitions, exercises, and games at the Orchard Community Church Cheer Camp are totally voluntary. I am aware of the nature of the activities and events for which I am registering my child, and I understand that accidents and injuries may occur as a result of their participation in such activities and events. Therefore, on behalf of myself and my child, I hereby agree to assume all risk related to said participation. I hereby waive, release, indemnify, and hold harmless Orchard Community Church, its agents, employees, and officers from any and all liability for personal injury or property damage which I or my child may incur, or which may hereafter accrue to me or my child as a result of participation in any of the activities or events conducted by, on the premise of, or for the benefit of Orchard Community Church. I further agree that this waiver, release, and assumption of risk shall be binding upon submission of this form. I also hereby agree to indemnify and hold harmless Orchard Community Church, its agents, employees and officers agains all claims, damages, losses and expenses, including attorney's fees, which they may incur as a result of my child's participation in the said activities/events. This waiver of liability does not apply to any acts of gross negligence, or intentional, willful or wanton misconduct.

I am submitting this form voluntarily, and by doing so I grant permission for my child to participate in Orchard Community Church activities and events and accept all risk associated therewith. It is my intention to exempt and relieve Orchard Community Church from all liability.
Here at MEGA Sports Camp, we want to take photographs of our kids in action as they participate in their various athletic activities... We promise that these photos will be used for promotional purposes only.
